- Is it possible to access this specific page using its original URL (when Permalink Manager is deactivated)?
- Is it possible to access this specific page after the custom permalinks is changed from “test-page-flat-5.html” to e.g. “test-lorem-ipsum-5.html“?

To answer your questions 1. No, 2. No.
BUT! when deactivating the plugin, I noticed something odd in the url.
There was a page type that was moved to a post type on my site. That moved post was a couple steps above in the hierarchy. That post must have been moved with a page still calling it its parent. That must not have played well with structure.
It just happens that all the testing I did I was picking that “test” parent (with the post parent), causing issues with every new page I made.
I’ve been able to successfully create new pages, using page only parents, without issue.
