Cancelled and failed orders recorded in conversion

The Pixel Manager doesn’t fire the pixel when an order fails.

It does however always fire the conversion pixels when a customer reaches the purchase confirmation page (except for failed orders).

If conversions are counted on failed orders it is likely that you have some other conversion tracking code installed, that doesn’t detect failed orders.
