Looking for free/inexpensive form plugin that can display entries on a page.

I'm looking to have a simple signup form on a small community site that also displays a list of signups for an event. I've started collecting signups using Formidable and just realized they charge $100 for the plan with this feature.

My form is very simple, it has space for the user to enter their name, two radio button questions, and an optional checkbox. We have 4 max events a year. There is no payment involved. I just want to display a list of names who have signed up. Is there an alternative that will allow this?
