Thank you very much for using our plugin. The issue should be related to the styles inherited from the theme active on your website. I tried to replicate the reported issue with the online demo and my local plugin copy. The controls are not visible. Please look at the screenshot image below:
Please let me know if the issue occurs when you press the preview button in the forms builder.
Could you please indicate the URL to the page that contains the form to check the styles loaded by your website?
