This could fail for a number of reasons. One possibility is that the built-in permalink formats are being overwritten by incorrect rewrite settings for custom post types or taxonomies. Another possibility is incorrect .htaccess/NGINX configuration.
Permalink Manager will not work if all of the pages were returning a 404 error prior to installing the plugin. To get the plugin to work, you must first ensure that the original permalinks are working correctly.
Could you send me a sample URL where 404 error is displayed?

To fix it, you just need to select any option other than the first one “Plain” in the “Settings -> Permalinks” section.
