It looks like it has something to do with the image picker which is used on both forms. When one of the image picker options is selected, we get the error message. When we de-select it, the form submits as expected. We’ve interacted with the other forms fields, and everything works fine. We specifically tested this on the self-evaluation page.

Sorry for that, yes there was an issue with the image picker. This is fixed in the release that was done a few seconds ago.
