I’ll try to do my best to explain what I’m looking for, as I’m not sure if anything like this currently exists. I’m looking for an app that will create a sentence or paragraph based on checkmark options that are selected or not selected.

* Sex:
* ☑ M
* ☐ F
* Past medical history (*typed*): \_\_cholangiocarcinoma\_\_
* Post-op day (*typed*): \_\_3\_\_
* Symptoms:
* ☐ Fever
* ☑ Nausea
* ☐ Vomiting
\^ \^ Typing this in and selecting these checkmark options will spit out something like:
*”****50*** *year old* ***male*** *with past medical history of* ***cholangiocarcinoma*** *is post-op day* ***3*** *complaining of* ***nausea*** *and denies* ***fever*** *and* ***vomiting****”*

Does this make sense? Anyone know if anything like this exists, or how I could go about creating something like this for a website? The idea behind this is for medical students who have to round on patients in the morning can look at a patient’s chart, fill in these details on the website I’m looking to make, and it’ll output something like the italicized text above, with the option to save as a PDF.
