Best way to run a simple shopping list for employee supply request?

We run a large print shop and I purchase a lot of consumables, from dozens of suppliers. In order to hand this task off to someone else, I have to get away from the tribal knowledge of supply requests emailed to me where I know what they want and where to get it, based solely experience.
Basically, I want the list of all 200 skus we order on one page. They each need a check box on the left of the item and a field to enter the quantity on the right. The user would simply check the items they want and add the quantities to the right. SUMBITting the request would either populate a spreadsheet or generate an email with the products checked and the quantities entered.
The list of products has four columns: SKU, desctription, UOM, Supplier. Using this, anyone receiving the email can immediately generate a purchase order for suppliers without having to look up where we buy each item from. A spreadsheet would allow me to sort by any of the four columns (vendor most importantly).

I’ve looked through shopping cart software as well as form plug-ins but I can’t find a simple shopping cart plugin that isn’t made for running full commerce sites, or a form plugin that allows me to display the product list based on a .csv file. The goal is for the four points of information to be displayed along with the quantity when the request is received after submitting. Any help pointing me in the right direction would be greatly appreciated.
