It looks like you are using NitroCache which may be removing the javascript required to show the campaign on your homepage.
You can exclude https://a.omappapi.com/app/js/api.min.js from being minified/removed, and it should fix the issue of the campaign not showing on the homepage.

Thanks for the tip. Is that an OptinMonster js code file? I disconnected NitroPack and the campaign started working on the home page. Thanks for that.
Why would the js file be removed from the home page but not the other pages?

Yes, that is the js OptinMonster uses.
Its possible that somewhere in the NitroPack settings its set to remove the js script from the homepage only and not other pages. I recommend getting in touch with their support to see if you can better control the exclusions.
