We need to build a multi-screen form that presents the 18 questions in an Adult ADHD Self-Report, & then displays a numerical assessment result total based on combined numerical values associated with each of with answer options.
I.e., Each question would have between 3 & 5 possible answers, each assigned as value. The answer options would be, Never, Rarely, Sometimes, Often & Very Often.
