Please open Developer Tools and go to Network tab. Once you are there hit form generation button and check the response in the network tab. Let me know if you see anything there.
My sincere apologies, Senol. I should have done the simplest thing first. I cleared cache everywhere except at the browser level. FAIL. Once I did that on Chrome, crisis averted. Everything looks fine now. Thank you so much for your quick response. I will, however, tuck away the Network tab check on Developer Tools for future troubleshooting tools. Thank you again.
