I’ve been having this issue on my site for a while.
It only appears to new users, or those who use change WIFI locations.
I have several security features like Wordfence Security, Solid Security Basic, Really Simple CAPTCHA, etc. But nothing has helped.
I even tried scanning my website for issues, but nothing came up.
I don’t know much about WP and how to find this issue… perhaps somehow getting an expert to look through my files to see what’s happening?

I can help, as you are using really simple captcha, it is asking for click to verify for a robot, you can deactivate it, if it not works than try to deactivate the other security plugins
