When what you describe happens it means the plugin is not working correctly. As explained in the docs, any sort of page caching prevents dynamic plugins like Blackhole from working. Normally the red/warning page is displayed only once to each bot. After that it’s just a simple “blocked” message that’s displayed. So if that is not happening, something is breaking things.
