Crocoblock form builder & appointments not pulling through custom fields?

Hello all,

First time using the form builder and JetAppointments. I’ve added a couple of extra columns to the database and created the custom fields pet\_type and pet\_weight. However, when the appointment is created it’s missing this data but when I check the form records it shows the data was sent.

Any ideas? Pics below.
