What causes the discrepancy between what a form [looks like originally]) (how I designed it and on the preview page) vs [how it looks on the page]) when published?
I get the same results using both “Website Code” and “WordPress Code” ([provided by the form creator]) using the “Custom HTML” block. Thanks for any help here.

Your theme obviously has styles that are being applied on the front-end but not on the backend. So your theme css is overwriting any styles you added specifically to the form
