Regarding the Booking Form not appearing, the issue that comes on your trips page might be raised due to the Primary Pricing Category not being set. So please set one of the Pricing Categories as a Primary Pricing Category by going to Admin Dashboard > Trips > Pricing Categories, i.e., Check here: Screenshot and verify the issue.
