Maybe the list the form is connected to has been changed or deleted?

In your WordPress admin please go to MC4WP > Mailchimp, and click on “Renew Mailchimp lists”

Then go to MC4WP > Form > Settings, make sure a list is still selected, and click save.

Now test the form again, did this solve the error?
