Thank you very much for using our plugin. Please, go to the menu option “Calculated Fields Form > Troubleshoot Area and General Settings”, tick the “Tick checkbox if there is an optimization plugin active on the website, and the forms are not visible” choice, and press the “Update” button. Then, go to WP Rocket and purge the website’s cache.
If the issue persists, please, configure WP Rocket to exclude jQuery from deferred scripts. Please, read the following WP Rocket article:
