Hi,
There are some settings to do those first 2 customizations in the paid plans, but not in the free version. When you look at the plugin settings (WP admin / Contact List / Settings), there are lots of settings and some of them are for the free version, and some for the paid plans (there is an indication on which settings are for the free / paid plans).
For the [contact_list_simple], you can show / hide certain columns from the plugin settings, Simple list tab.
