What form are they using? WP-Forms?
How would I incorporate this into my hero banner like they have? Any recs for plugins etc. I want to create a lead page exactly like that for our business

It’s just a standard form which can be created using any standard form plugin or manually built.

And some css to adjust it with colours of logo.
