How do I know if a file is added by a hacker or regular maintenance job?

I have Sitelock and also the free version of Wordfense. It has high warnings for the following

Now,i know little about the backend of things. I have a theme I update when it’s time. But, other than that I have no idea. Is there an option to stop people from uploading things to the public folder?

Is there a way to see what it is? Would it hurt if I delete it?
