I need help with featured image and revisions.

I have an installation which is using a custom post type “cars”. When I edit a new car and add a featured image, then save, everything appears to be fine. Returning to edit the car after a few weeks, I can see that the featured image is missing. I have enabled revisions for custom post types, and I can see in the oldest revision that I have added the featured image. Then there is no activity recorded with regards to that, but there are more revisions for other parts of the listing. Then I have to manually add the image one more time after I have realized that it is gone. Checking the revisions there is no record of activity related to removing the image. Then I manually add it a second time, and the latest revision records that as well.

Would anyone have experienced a similar behavior before? What could it be? How could I make it work as it should? Meaning, once you set the image, it stays there until you manually remove it or update it with a different one.
