I guess there must be some medical booking theme on the market with this feature installed. Otherwise you need PHP and css development.
Just right click and inspect how they do it. Then use the html and css and your own PHP to handle the search however you intended.
For the top half, you could probably use Search & Filter Pro. I don’t think it would handle the bottom half though.
