Need help finding a plug in that acts as a help center

I need a page on a current build that reports well being and concerns.

There would be a series of predefined answers/questions that the user clicks and eventually ends up (depending on selection) to a certain charity or help line number.

I have done something similar with contact form 7 but was hoping for something less clunky and had a bit more flair.

Can anyone recommend something that can do this?
