I am racking my brain looking for a low-cost or free solution to this problem.
I need a forms-type WordPress plugin to generate a pdf report with the following requirements.
1. Multipage
2. Pictures included
3. Default text along with fields that are filled in.
4. Repeater fields
I’d like to be able to collect data in the field, and have the reports generated automatically.
I am conducting engineering surveys, and always have to duplicate work adding hours upon hours to my work.
I have looked at google forms, WordPress plugins usually want me to buy hundreds of dollars of add-ons to do this, and I never got a complete solution.
Would really appreciate it if someone can point me to the right plugin or solution.

Have you considered Google Sheets with automated functionality, webhooks… to get the data and convert to a pdf file?
I have used Jotform.
It has those features you’re mentioning. There’s the free version that you can use to test it.
You can embed their forms on WordPress or any website. For downloading the PDF report, you’d need to do it through the Jotform website.
