This is most likely happening because you saved an invalid translation text on the Site Reviews > Settings > Strings page. Did you forget to include the %s placeholders in your custom text?
This is how my Strings page looks, am I doing something wrong?
If you aren’t using the Strings page in the settings, then which plugin are you using to translate the form?
Also, have you added any custom code snippets to your website to change the functionality of Site Reviews?
Loco translate was used for plugin translation.
There are no other code snippets on the site.
