1. Are you using reCAPTCHA with WPForms?
Since you are using the “WPForms” plugin style in the Site Reviews settings, it’s possible that WP Forms is adding the reCAPTCHA to your page and not Site Reviews (since the Site Reviews form includes the WPForms CSS classes).
You can test if this is the case by changing the Plugin Style in the Site Reviews settings back to the default style.
2. Site Reviews automatically uses the Gravatar service when avatars are enabled in the Site Reviews settings.
When a review is submitted, Site Reviews will check if there is an gravatar saved for the submitted email. If there is, Site Reviews will save it to the review. If not, it will use the “Fallback Avatar” setting.
Because the avatar is saved to the review, it will not update the avatar when the gravatar image is updated.
If you want to always display the latest gravatar avatar for the email or user who submitted the review, then you need to enable the “Regenerate Avatars” setting.

You guessed it right, I was using the WPForms style, resetting to the default fixed the problem.
As for the avatars, my gravatar (the one I am sure exists) doesn’t shows up. However, since my users seem to be afraid to leave an email address, I have decided not to show the avatar result. But since I want to figure things out, I’ve tried putting a dummy email address on people who haven’t filled it in, and I’ve found that the feature really works, other than the fact that if someone doesn’t have an email field, the image of the avatar falls on the first generated (ie, the avatar for the first guy with an email). The “initials” fallback on the other hand always works, regardless of the email.
