Plugin for user confirmation for general terms and conditions

In our company, we send automated e-mails to new customers that we register in our ERP system (not connected to our website). To get them to accept our general terms and conditions as well as to agree to receive newsletters, I am looking for a plugin that can

a) create individualised links to a landing page (individualised so the user than clicked it can be identified)

b) show a landing page with a confirmation form

c) save who clicked and who did not click in a database, only accessible by wordpress users.

This database will then regularly be used to update the information about terms & conditions and marketing in our ERP system. Does something like this exist?
