Plugin for user upload/download management?

Hello there,

I wish to create a website which basically serves as a repository for user uploaded files. The functionality I need is: user registration, ability to upload a file of a specific type, ability to customize the upload form to include certain predefined fields (e.g. “Name of model”, “Model year”), and ability for all users to be able to download, rate and comment on specific files.

I have a domain, and WordPress is installed, but I’m very new to all of this so I’m not sure where to go from here.
