Quiz app with a custom URL.

I want to add a WordPress Quiz app with a custom URL. I want to use a Quiz as a template and have a second tier with Quiz\_Mary, Quiiz\_John, …. the person sharing the quiz would automatically have their name appended to the end of your quiz’s URL.
