I have a paid addon Save Results Pro that does exactly that 🙂

With this addon, you can request the quiz taker’s name and email (or any other information as well, if needed). There are also two features it has that might be relevant to you.

The first is where the form is located. “Before Quiz” means that the user needs to fill out the form (example: name and email) before being able to take the quiz. “Before Results” means that the quiz taker answer all questions, but won’t be able to see their results until they fill out the form.

The second is email. There is an option to send the results via email, with an additional option to send the results via email to the quiz taker. This is especially cool because you can also add custom content to the email so that the email is customized for your site.
