Seeking Help: Replicating a Complex Form Design in WordPress

In my research so far, I’ve come across formidable and some code snippets for the logic in the form, but I’m struggling to replicate the same styling or integrate the calculator.

My question is, are there any plugins or methods you recommend to achieve a similar form to the one on the referenced page?
