Which plugins to use? Accessible forms/tables to users in front end?

Hello wp colleagues!

I need to add to my wp personal website (research purpose, not commercial) the following functionality:
– users should be able to see, add and edit records to a predefined form (this should contain general details of a patient – name, ID, location, etc.)
– users should be able to open any patient and see, add or edit records to another form corresponding to hospitalization (date, symptoms, lab values, etc.)

Please tell me which plugin should I look into! I couldn’t find anything remotely useful..
