I am looking for a plugin or a way to create this process with WordPress and Elementor:
1. User takes a questionnaire with 3-5 questions
2. Based on their answers they either go to a page saying they are not qualified, OR a page where they schedule an appointment and pay for it. But there will be 2 options
1. Pay with insurance (this will take no payment and just book the appointment)
2. Pay without insurance (this will take their payment info and they will make a payment for the appointment)
3. They get a confirmation email with their appointment date/time
