Wpform issue

I created a newsletter subscription form using wpforms. I only added the "name" and "email" field. When I insert the form in my spectra page editor also it shows the name and email field only. But when I published my page..there is an additional field named NameEmail as shown above. This is not supposed to be there. Why is it showing as such and how can I remove it? Thanks in advance.
