Do you know a form that supports dynamic questions – questions change based on the post / article

I have a website and on each page use a lead form.

The lead form is a multistep form with 2-3 qualifying questions.

On each page the qualifying questions are somewhat different. The final contact page is always the same.

Right now I have to create a new form for each page but that is lots of work.

I am therefore looking for a plugin that could take the questions from custom fields on the post.

Any ideas?
