I found the answer to my first question under the Comments menu option. Which also confused me a little more since the one I “approved” does not appear there. It must have been for a different approval. I am sorry to clutter your support forum, but I will leave it here for others who may be looking for the same information.

Comment reply subscriptions do not have to be approved by you and therefore there is no approval in the backend. But however there is a list of subscriptions in the comments section. People can also decide to remove their subscription if they don’t want to keep it – a link for that is generated and sent via e-mail to the subscribers when they get a notification sent.
