Thank you for the topic.
In order to achieve your desired result and set the Information Form before the quiz, please head to the Quizzes > particular quiz > User Data tab, and set the Information Form option as Before Quiz.
Then, take the Name and Email custom fields from the Available Fields and insert them into the Active Fields. You can make them required.
By this, the users will need to fill in their names and email addresses to start the quiz.
