I don’t know if it makes any difference but I have installed the Autocomplete for Calculated Fields Form as well and today I have installed the Appointment Hour Booking plugin.
I didn’t touch the calculated fields form or the page in a few days though.

But it is not including the script tag with the form_structure_1 variable.
Could you please provide the website information? You can get it through the “Tools > Site Health” menu option, and by pressing the “Save to clipboard” button in the “info” tab.

I just followed your instructions and seems that the conflict is caused by Appointment Hour Booking plugin. When I deactivated it, my calculated fields form showed up.

Have you deactivated the AHB plugin? I visited the form page ( https://homevetvisit.uk/?cff-form=6 ) and it was loaded properly.
