Just done that, still the same result.
Is there anything I need to configure in ‘On-site Product Review Form‘?
No, you don’t need to configure anything. It should just work.
A similar problem was reported by another person. In their case, it was caused by some plugin for “performance optimization” that they installed on their website. After they deactivated that “performance optimization” plugin, everything started working correctly.
Please follow these troubleshooting steps:
- Deactivate any 3rd party plugins except WooCommerce and our plugin.
- Verify if the issue is resolved
- If the issue is resolved, re-activate the plugins one by one and re-check the issue to confirm which plugin is causing the problem

Awesome, thanks for the tips.
It turned out to be perfmatters which had cusrev unloaded from the frontend (except for the ‘products’ pages, of course), and this wasn’t an issue until we switched to on-site review which then it would require the resource/modules to be load.
I should have it fixed soon. Thanks heaps.
