Sending emails to users or resetting passcodes dont work all the time

When we make a new user and check the box to send user alert about new email

Or when we reset the passcode for the user and send them the email

Or when the user uses the reset passcode link on their own, sometimes the emails do not go to these people and sometimes they do

I used the Check & Log Email plug to test and the email test was sent to me just fine, we have a gravity forms contact form on our site and that seems to work fine as well

How would we go about fixing this issue?
