I handle all the maintenance on sites I build of course but I’m seeing many people don’t. I feel like you’re leaving money on the table.

Definitely a no! If you can handle it, you should be compensated separately for that.
Some customers don’t understand the need for maintenance. When the site gets a problem, they ask the developer to fix it and of course he won’t if he’s not being paid, then he gets a bad name.
Its a feature of free WordPress, free themes and free plugins – customers expect free maintenance.
The people that dont manage the maintenance and charge for it, will have trouble scaling theor business
If you’re managing web maintenance you’re no longer (just) a designer.
Like you said, it’s money up for grabs. The moment you take on that responsibility, you take on that responsibility.
We charge $150/mo for basic hosting, maintenance, and support.

I don’t know any designers who have the technical capability though. The designers who do it come to me, a web developer, when it’s over their head.

You should simply quote the work done with breakdown with the maintenance as a recurring annual fixed fee or as an hourly rate. You may include a 1-3 month maintenance as part of the design fee for only fixing issues.
That’s my entire business model. Managing after completion is steady income!
