Suggestions for improving TTFB?

Our company website has some major issues with performance scores in various metrics, mainly on mobile:

I’m mainly focusing on Pagespeed Insights (pagespeed.web.dev) at the moment but I have also checked various other tools like GTMetrix, Pingdom, etc.

It seems to mostly stem from a very poor TTFB score. But the FCP score could definitely use some improvement as well.

What I don’t understand is how google is deriving this TTFB score, and why. When I test the website in Lighthouse (in Chrome), even with the Slow 4G setting (1.6Mbit, 150ms latency), I usually get about 900ms response time if the page is retrieved from the CDN cache. And about 1200ms response time if the CDN cache is bypassed. (So the server has to generate the page through PHP). With ideal settings (no speed throttling, CDN cache is HIT, the response time is about 500ms.

Does this just mean that all our mobile visitors have extremely poor network speed? Or does this page somehow only include the visitors that bypass the CDN cache?

I have made a lot of adjustments to improve the CDN hit rate from 40% to 70% last week, but it doesn’t seem to have had any effect on the scores.
