For multiple forms in different languages, we recommend using our premium version that allows creation of different forms. Some data of the form is set in the code which is not possible to translate.
Alternatively, you can use a plugin like “Contact Form 7” which is a free plugin. With this you can create a form for each language and integrate it with our plugin to send the subscriber data to MailChimp.com
