The first piece of advice would be to clear the cache of your optimization plugin Litespeed. There may still be a remnant of a previous plugin in there, which is causing the output.
The form itself comes from the Formidable plugin. You could deactivate this plugin completely to prevent the output. However, this would remove all forms on the page.
The output itself is apparently controlled by a popup function. Have you ever used the “Popup Maker” plugin? It almost looks like that. If so, this plugin has somehow left a residue here that should probably be cleared by emptying the Litespeed cache.
