I noticed the form on my WordPress website is not showing anymore for some reason. All I see is a piece of code where the form is supposed to be. Does anyone know how to fix this?

Well you got an issue with the wpforms plugin
Add a gutenburg shortcode block where you want that form to be and put that code in there.
Silly questions, but is WPForms installed and activated?
