Your form seems to be working fine in the form tester, which is using CF7 version 5.7.1 and Conditional fields version 2.3
So if you still have problems on your website after updating both plugins, it means that there is a problem with your theme or a third party plugin. Not everything. If you switch to “Multimedia” in the form, the fields E-Mail and Name don’t appear. But this works with CF 5.6.4.
And in the meantime I created a clone of my site and this issue appears also when all plugins are deactivated and a WP standard theme is activated.
Now I’m really confused. What can I do?

Thanks! A hCaptcha plugin causes the issue with missing fields during checkout. Now all is fine!
