Name and phone number not appearing in email after submission

Resolved. I simply had to put the proper tags into the mail tab.

I have a contact form designed to collect the first and last name, phone number and email of participants. When the forms are submitted, the info shown is found below. I see the email, but no other information shows up. For this contest we need the phone numbers so I was hoping to find out how to make it so they appear in the emails we receive when participants submit the form. Below that photo is what I have in my forms both the mail tab and the form tab. Any insight as to how I can fix this would be greatly appreciated, I’m kind of a noob.
