This seems to be a known issue specific to your hosting structure (mostly on Flywheel installations) The default path is causing a conflict and causing the stylesheets to not load and hence the radio boxes are not working.

Please update the path for the “File Upload Storage” setting under Forminator Pro > Settings > Data > File Upload Storage to the following:
/wp-content/uploads/forminator/

Please also clear all of your caches after that including your server side caches and check if it helps. Our Forminator team is working on a fix which will be released on the upcoming versions of the plugin, however we don’t have any ETA to provide at the moment.

Thank you, that worked. I see now in the console where it was pulling in that /www/ pathway. Appreciate the quick response.
